Admissions Coordinator - Added 5/9/19

Reports to: Admissions Director

Job Purpose Summary:

The Admissions Coordinator is responsible for providing information and support for clients, families, and treatment providers, while facilitating and evaluating the clinical, medical, and financial requirements of the admissions process.

Responsibilities Include:

  • Answer Admissions calls using a professional and courteous manner, during work hours and afterhours.
  • Follow up with all Admissions opportunities, AMA clients, and refer-outs.
  • Complete over-the-phone assessments according to criteria set forth by Victory Bay.
  • Complete insurance verifications.
  • Ability to speak intelligibly and accurately about our program and services offered.
  • Coordinate and schedule new intake dates and times.
  • Coordinate transportation for new intakes.
  • Work well on a team and maintain a high level of organization.
  • Complete all necessary documentation in a timely manner to keep electronic records up to date.
  • Obtain previous treatment center’s clinical and medical records.
  • Assist in coordinating a referral process.
  • Conduct Admissions process.

Job Type: Full-time

Hours: 8:30am-5:00pm M-F, after hours phone availability

Required Education: Bachelor’s Degree in a related field or related experience preferred.

Required Experience: 2-4 years’ experience working in a Drug & Alcohol Treatment Facility, 1-year Substance Abuse Intake required.

Salary Range: $40,000-$45,000 /year

Evening Therapist PT/FT - Added 5/9/19

Job Purpose Summary:

The primary therapist responsibilities include leading didactic and therapeutic groups, collaborating with case management, and providing individual counseling to client case load.

Responsibilities Include:

  • Interface with the director and other program staff in decision-making process involving clients and relay all findings during clinical staffing.
  • Utilize screening and assessment tools such as the PHQ-9, Columbia Scale, BAM, and related tools in the constructing the client’s treatment planning process.
  • Assess the counseling needs of the client
  • Assessing client for substance abuse diagnosis using DSM criteria in determining appropriate level of care based on ASAM criteria
  • Obtain previous records that are relevant to the current treatment episode
  • Preparing and providing the substance abuse counseling and co-occurring portions of the client treatment plan
  • Development of the client discharge/continuum of care plan to ensure transition to the appropriate levels of care.
  • Documenting all counseling and education services, assessments, reassessments, referrals and follow-up in the client’s clinical record.
  • Contact and coordination with referral sources, (i.e. legal, school, employer, etc.)
  • Facilitate needed interventions for crisis-orientated issues utilizing a multi-disciplinary team approach for assigned clients.
  • Facilitate group sessions, following the curriculum of the program.
  • Have knowledge of 12 step recovery programming and curriculum integration
  • Be proficient in the use of Microsoft Outlook, Excel, Word, Power Point and OneDrive.

Job Type: Part-time or Full-time

Hours: Mandatory hours include Monday, Wednesday, and Thursday evenings 5:30p-9:30p. Additional hours are available during both day and evening.

Requirements:

Must possess one of the following certifications in the State of New Jersey

  • LCADC
  • LCSW or LPC
  • LSW
  • LAC
  • CADC

Care Coordinator - Added 5/17/19

The Care Coordinator works directly with the external billing service for certification and re-certification of insurance benefits throughout the patient’s stay and assists the treatment team in understanding the insurance company’s requirements for continued stay and discharge planning. The Care Coordinator is responsible for having a thorough understanding of the patient’s treatment through communication with the treatment team. The Care Coordinator advocates for the patient’s access to services during treatment team meetings and through individual physician contact.

 

Job Summary

  • Complete initial BioPsychoSocial assessment.
  • Complete concurrent reviews for caseload to extend clients’ length of stay.
  • Collects and records all information necessary for admission and extended stay review.
  • Conducts random continuing-stay reviews to ensure patients in treatment require and are benefiting from the services they are receiving.

  • Welcome new patients and guide them through the initial intake process.
  • Obtain all applicable consents, financial agreements, and ROIs.

  • Act as primary point of contact between external billing service and Victory Bay Recovery Services
  • Communicate with the insurance department if a client coverage or limitation issues exists.
  • Communicate level of care authorizations to clinical team.
  • Facilitate peer to peer reviews as necessary.
  • Collaborate with clinical care team to obtain payor request for specific treatment, follow-ups, clinical information and individualized care.
